I imagine the density of NYC factors into it, but it isn’t that simple. San Francisco has the next highest density. It was an early hot spot but got things under control without overwhelming hospitals.

New Orleans has been hit hard.

Albany Georgia is one of the worse spots per capita. That is not exactly a dense metropolis.

Right now, metros are the hot spots. I worry about if this will really hit rural communities eventually. Smaller risk of infection. But as an example smaller towners in the tri-state area drive all the time to NYC, Philly, NJ. All it takes is for a few to get infected to spread it fast. Social distancing has especially been effective at keeping these people from traveling to the cities but what happens when it's lifted? In these small towns, it wouldn't take much at all to break the hospital system which are all already struggling for resources and probably last in line to get protective equipment.
