It's looking like my want for a negative test is double crossing me. I appreciate the feedback.

Yeah that’s down around where I grew up. They definitely didn’t take it very serious early.

Now it’s hitting extremely hard down there

The Midwest is definitely being hit the hardest, but there's areas so over the country seeing a big increase in cases.

Updates on therapeutics:

--Convalescent plasma failed a Phase II RCT in India
--Remdesivir was given FDA approval for all patients over 40kg based upon trials of 1000 patients each, but the information from SOLIDARITY did not demonstrate the same benefit with a larger cohort (~2700 patients)
--IDSA guidelines recommend remdesivir use for patients with moderate-severe COVID, with five day courses for patients not requiring ventilation, and 10-day courses for patients on mechanical vents or ECMO
--Dexamethasone is recommended for patients with moderate to severe disease
--Hydroxychloroquine is not recommended
--Hydroxychloroquine and azithromycin are not recommended
--Tocilizumab is not recommended outside of a clinical trial
--Kaletra is not recommended outside of a clinical trial

That's a whole lot of disappointment. :-)

Anything in clinical trials that DOES look promising?

From the outside, I understand your disappointment, but consider that it took six years for any FDA-approved therapy for HIV, Hep C has only been realistically curable in the last decade, and there is still no treatment for many viral infections beyond supportive care.

With that said, the demonstrated mortality benefit from dexamethasone and the potential morbidity benefit from remdesivir are promising and are one of the reasons why death rates are less severe than the spring despite an uptick in confirmed infections.

A magic bullet was always incredibly unlikely given the nature of viral infections (vs. bacteria or fungal colonization). The Oxford vaccine trial seems to be progressing well. I wouldn't despair.

That's a very reasonable way to look at things. Thank you for your input. As always, it's much appreciated.

Viral therapeutics are just a bitch to make. There’s a reason those little fuckers are so hard to deal with.

The mAb are still the beat option I think but they’re hard to make. Just gonna have to grit thru and hold on right for another couple months
