A lot of is that he is older now and also at his heaviest. He is around 225-230 now. In Rocky IV he was mid 170’s.

HGH does not make a grown mans head grow bigger. It can thicker some area of the skull so that might be some of it.

Robert de Nero’s head looks bigger now than when he was in his 20’s and I am confident he does not use HGH.
